Symptoms 

Sometimes two SolusVM VPSes provisioned from WHMCS are created with the same IP address or same vserver ID

Cause

This is SolusVM bug with ID #SVM-3131 which was fixed in SolusVM version 1.25.14

Resolution

Update SolusVM to the latest mianline version.
